Three steps

Install

Open the installer and follow the steps. It places the plug-in in Illustrator's Plug-ins folder.

Restart Illustrator

Then choose Window › Creasekit › Box to open the panel.

Make a box

Pick a box type, type the sizes and click Create dieline. Read the docs.
